Faculty and Staff: Life Insurance Checklist

There is still time to take advantage of this one-time opportunity through MetLife to enroll in or increase your optional life insurance without answering health questions and/or having a medical exam (even if you have been turned down before due to health reasons).  This special opportunity is only available from Nov.1-30, 2016 during the MetLife's open enrollment.

  • Review the October For Your Benefits article on page 4: “Life Insurance: Your one-time opportunity in November will guarantee coverage starting in 2017”
  • Review the MetLife Frequently Asked Questions on the MetLife website.
  • Use the MetLife Life Insurance Calculator to determine how much life insurance you need.
  • Review the Support, Planning and Protection Advantages MetLife offers. These benefits include grief counseling, face-to-face will preparation, solutions for continuing workplace coverage and more.
  • Register for the MetLife MyBenefits  portal to view your life insurance coverage.
  • Enroll in or increase life insurance for you and your dependents. If you wish to continue child life insurance and Accidental Death and Dismemberment (AD&D) insurance for your spouse/state-registered domestic partner and child(ren), enroll no later than November 30, 2016. Or, complete the MetLife Enrollment/Change Form for Enrollment Period from November 1, 2016 – November 30, 2016 form and submit it to MetLife (address is on the form). MetLife must receive the form no later than November 30, 2016.
  • Even if you are not enrolling in or changing your life insurance, you must name your beneficiaries as that information will not transfer to MetLife.
