How do I make changes during the PEBB Program annual open enrollment?

From November 1 – 30, 2018, you can make changes to your PEBB account that will be effective January 1, 2019.You can make your changes online in My Account

Online changes: PEBB My Account portal

Go to My Account on the PEBB website. You can:

  • Change your medical or dental plan(s).
  • Waive enrollment in medical coverage if you have (or are enrolling in) another employer-based group medical coverage, a TRICARE plan, or Medicare. Note: Coverage through the Benefit Exchange is not considered group coverage.
  • Remove dependents from your PEBB medical or dental plan.
  • Attest or reattest to the tobacco use premium surcharge or the spouse or state-registered domestic partner coverage premium surcharge. For more information on these surcharges, see your October For Your Benefit newsletter.
  • Note: You do not need to attest to the tobacco use premium surcharge during the PEBB Program open enrollment if your or your dependent"s tobacco use status has not changed.
  • Print and save your confirmation page when you"ve completed your changes. Updates made online do not immediately display in My Account; please wait 2-3 business days to make any additional changes.

Paper Forms

To add an eligible dependent who is not currently enrolled on your account, submit the 2019 Employee Enrollment/Change form and proof of their eligibility(valid dependent verification documents) with your enrollment form to your benefits office no later than November 30, 2018. You must provide these documents before your dependent can be enrolled under your coverage.

Visit the PEBB Forms &publications webpage to find the 2019 forms you need to make your changes.

  • Use the form to change medical or dental plans, add or remove a dependent, or waive medical coverage.
  • If you are not making plan changes, but need to attest to the spouse or state-registered domestic partner coverage premium surcharge, use the 2019 Premium Surcharge Help Sheet to determine if the premium surcharge applies to you, and the 2019 Premium Surcharge Change form to attest.
  • Submit your forms to Western"s benefits office. Western must receive your forms no later than November 30, 2018.
