  • If you prefer a paper form, use the 2019 Employee Enrollment/Change form. It will be available on the PEBB Forms & publications webpage beginning November 1.
  • How do I enroll in the Medical Flexible Spending Arrangement (FSA) or Dependent Care Assistance Program (DCAP)? – Enroll or reenroll in the Medical FSA and/or the DCAP online through the Navia Benefit Solutions website or by completing a paper form, also available on Navia’s website.
