Employees must re-attest to spousal premium surcharge

Western Washington University employees with a spouse or registered domestic partner who is enrolled in medical on the employee's PEBB account must re-attest to the spousal premium surcharge for 2015.

Due to the change in UMP Classic benefits and premiums for 2015, employees must determine whether the spousal premium surcharge will apply using 2015 plan information, making comparisons against the UMP Classic plan even if enrolled in a different PEBB medical plan.

To re-attest to the 2015 spousal premium surcharge, employees should log into My Account on the PEBB website and complete the six questions. Answering “No” to any of the questions means the $50-per-month spouse premium surcharge does not apply. Otherwise, employees should complete the 2015 spousal plan calculator using the summary of benefits and coverage from the spouse's or registered domestic partner's employer.

Employees also may re-attest by submitting a 2015 Premium Surcharge Change form to Human Resources by Nov. 30. The six questions are available in the 2015 Premium Surcharge Help Sheet on the PEBB website.
