Employees: Are you enrolled in Uniform Medical's Consumer-Directed Health Plan (CDHP)?

HealthEquity (HEQ) is the vendor who manages the health savings accounts for members enrolled in the UMP CDHP plan. The HealthEquity customer service number dedicated to UMP CDHP members has changed to 844-351-6853. HEQ is re-issuing new bank cards with the updated number on the back. These cards will be sent to all members currently enrolled in the UMP CDHP plan.

When HEQ prints the new cards on December 16, 2019, the old cards will not work anymore. If members incur qualified medical expenses between December 16th and the date their new card is received, they can request reimbursement from their online account with HEQ; or, call HEQ customer service toll-free at 844-351-6853. Members who are already set up for electronic deposits will receive reimbursement within 2 – 3 business days of their request. Members who have not signed up for electronic deposits will receive reimbursement via a check mailed through the United States Postal Service (USPS) within 7 – 10 business days of their request.
